Senior PCN Pharmacist

• Prioritising patient care, seeing patients with one or multiple medical problems where medicine optimism is required, and undertaking clinical medical reviews for patients, ensuring patients receive the best use from their prescriptions.
• Working closely with GPs, and other practicing heath care professionals where appropriate, answering any patient medicine questions they have.
• Undertaking clinical audits of prescribing in areas directed by the GPs, communicating results and implementing changes.
• Managing the repeat prescribing reauthorisation process by clinical review of patient requests in line with review dates, and flagging those needing review. 
• Training and developing the primary healthcare teams on therapeutics and medicines optimism. 
• Ensuring legal compliance within the pharmacy, reviewing labels, doses, and securing inventories-controlled drugs in line with regulations. 
• Offering health-screening including blood-pressure monitoring, diabetes screening flu and other vaccine administration.
• Providing regular updates and monitoring of KPIs to the operational management team. 
• Resource planning to meet service and contractual requirements.
