As a Senior PCN Pharmacist, you'll be at the heart of local healthcare, providing expert clinical support, building strong relationships with patients and healthcare professionals, and helping people live healthier lives.

One day you’ll be carrying out structured medication reviews for patients with complex needs. The next, you could be supporting GPs with prescribing decisions, leading clinical audits or helping develop teams through training and mentoring.

You’ll use your clinical expertise and patient-centred approach to deliver high-quality care—every time. As a trusted healthcare professional in your community, you’ll help people understand their treatment options and get the very best from their medicines.

As a Senior PCN Pharmacist, you’ll be at the heart of patient care—bringing together clinical expertise, collaboration and medicines optimisation. You’ll support people living with long-term conditions, carry out clinical medication reviews, and work closely with GPs and wider healthcare teams to improve outcomes.

You’ll also respond to prescribing queries, manage repeat authorisations through clinical review, and use audits to drive real improvements in care.

And your impact doesn’t stop there.

You’ll support and develop primary care teams through training and coaching, share your expertise, and help shape better ways of working. From delivering services like vaccinations to ensuring the highest professional standards, you’ll play a key role in delivering safe, effective care.

You’ll also help monitor performance, contribute to key priorities, and support the planning of services that meet both patient and community needs.

About You

This is an important clinical role, so experience, confidence and compassion are essential.

We’re looking for a patient-focused pharmacist who is passionate about delivering high-quality care and making a difference within their community. You’ll be comfortable working with patients, GP practices and wider healthcare teams, using your expertise to support safe, effective and person-centred care.

You’ll also be someone who enjoys building relationships, sharing knowledge and helping others develop. Someone who can think clinically, communicate clearly and remain focused on what matters most: improving outcomes for patients.

You’ll likely bring:

• A Master’s degree in Pharmacy (MPharm) and current GPhC registration

• Strong clinical knowledge and confidence delivering advanced pharmacy services

• Experience working with patients and multidisciplinary healthcare teams

• Excellent communication and relationship-building skills

• The ability to make sound clinical decisions and manage competing priorities

• Experience supporting or developing colleagues

• A proactive approach to learning, service improvement and patient care

• A genuine passion for community healthcare and medicines optimisation
