About the role:

Grow your impact across primary care

What if your next role gave you the opportunity to make a real difference to people's health every day?

At Lincolnshire Co-op, our pharmacies are much more than places where prescriptions are dispensed.
They're trusted healthcare destinations where patients come for advice, support and expert clinical care. Whether it's helping someone get the most from their medicines, supporting long-term health conditions, carrying out clinical reviews or working alongside GP practices, our pharmacists play an important role in improving health outcomes across our communities.

As a PCN Pharmacist, you'll work as part of an evolving primary care network, supporting medicines optimisation, improving prescribing safety and helping to deliver joined-up care across General Practice and Community Pharmacy. This is a role where you’ll grow your clinical confidence, develop your experience and contribute meaningfully to local healthcare delivery.

Develop your clinical practice across a connected system

Every patient interaction is an opportunity to learn, support and make a difference

No two days are ever quite the same.

One day you might be carrying out structured medication reviews for patients managing long-term conditions, while the next you're responding to prescribing queries, supporting audits or working alongside colleagues to improve medicines use across the practice population.

Whatever the challenge, you'll apply your clinical knowledge, professional judgement and patient-focused approach to ensure safe, effective and personalised care.

You'll build strong working relationships across your network, collaborating with GP practices, community pharmacy teams and other healthcare professionals to help patients achieve the best outcomes from their medicines.

Support better outcomes through medicines optimisation

Behind every prescription is a person who needs the right support

Your work will combine patient-facing care, medicines optimisation and professional collaboration. As a PCN Pharmacist, you’ll:

• Prioritise patient care by supporting patients with one or multiple medical conditions where medicines optimisation is required
• Undertake clinical medication reviews, review monitoring needs and produce recommendations for senior clinical pharmacists, nurses and/or GPs on prescribing and ongoing care
• Work closely with GPs, community pharmacy teams and other healthcare professionals, answering medicines-related queries and supporting safe, effective prescribing
• Undertake clinical audits of prescribing in areas directed by GPs, feeding back results and implementing changes with the practice team
• Manage the repeat prescribing reauthorisation process by reviewing patient requests, monitoring review dates and flagging those needing further clinical review
• Support signposting and referrals to community pharmacy and other healthcare professionals where appropriate
• Help practices achieve Quality and Outcomes Framework indicators and support the development of services with medicinal components
• Maintain accurate documentation, ensuring interventions are recorded and coded appropriately
• Use the Electronic Prescription Service tracker correctly to support claims and manage processes effectively
• Train and support primary healthcare teams on therapeutics and medicines optimisation
• Ensure compliance with professional, legal and regulatory requirements
• Offer health screening services, including blood pressure monitoring, diabetes screening, flu vaccinations and other vaccine administration

About You

Bring your skills — and build your confidence

This role is ideal for a pharmacist who is looking to grow their clinical experience within a primary care setting. Curiosity, a willingness to learn and a patient-focused mindset are just as important as your existing knowledge.

We’re looking for someone who is motivated to develop, enjoys working as part of a team and is keen to build their confidence in a PCN environment. You’ll be comfortable supporting patients, collaborating with GP practices and contributing to safe, effective prescribing.

You’ll also be approachable, organised and able to communicate clearly with both patients and colleagues. Someone who is developing their clinical judgement, can manage their workload effectively and is committed to improving outcomes for patients.

You’ll likely bring:

• A relevant pharmacy qualification, current GPhC registration and eligibility to work in the UK
• Confidence providing expert health advice and delivering advanced and enhanced pharmacy services
• Experience or awareness of common acute and long-term conditions likely to be seen in general practice
• Prior experience in general practice and experience using SystmOne would be desirable
• An understanding of pharmacy governance and how to apply this appropriately in the workplace
• The ability to engage with Patient Participation Groups and involve them in the development of the role and practice services
• A professional and approachable manner, with the ability to build and maintain dependable working relationships
• Excellent communication skills and the ability to deliver high-quality face-to-face interactions
• Enthusiasm and drive to champion your own learning and development
• A commitment to helping create a continuous learning culture within your team
• A flexible approach to ways of working, with a full driving licence and access to a vehicle for business use
• Please note, this role is subject to DBS clearance

A role that supports your development and wellbeing

This is a great opportunity to develop your pharmacy career and make a difference to the lives of our communities. Alongside working for a growing co-operative business, you'll benefit from:

• A pension scheme with up to 12% employer contributions
• Generous colleague discount across our family of businesses
• Annual discretionary colleague bonuses to reward you for your hard work
• Reimbursement of GPhC fees in full
• Access to a wide range of professional development opportunities
• The opportunity to work across General Practice and Community Pharmacy, supporting integration between services and improving access to healthcare
• Up to 39 hours per week, with the role reporting to the Pharmacy Care Lead Pharmacist
• Wellbeing support and opportunities to get involved in community initiatives
